Кортикальные дисплазии как эпилептогенные поражения

Аннотация

Кортикальные дисплазии представляют собой пороки развития коры, являющиеся частой причиной фармакорезистентной эпилепсии. Они часто выявляются в качестве патологического субстрата, лежащего в основе МР-негативной локализационно-обусловленной эпилепсии. Приведены основные данные о механизмах формирования дисплазий, различные классификации данных поражений и эпилептогенности различных типов дисплазий. Обсуждаются значение мультимодальной диагностики и инвазивных нейрофизиологических исследований в дооперационном обследовании больных, а также актуальные вопросы планирования хирургических вмешательств при различных вариантах эпилептогенных поражений, обусловленных кортикальными дисплазиями. Представлены различные методики резекционных и деафферентационных вмешательств. Рассматриваются вопросы выбора тактики хирургического лечения, являющиеся предметом дискуссий.
