Глиобластома остается самой распространенной первичной злокачественной опухолью головного мозга у взрослых. Смертность пациентов с глиобластомой остается самой высокой среди всех онкологических больных, приближаясь к абсолютной. На современном этапе стандартный протокол лечения глиобластомы основан на комбинации методов и включает адъювантную химиотерапию и радиотерапию после максимально безопасной микрохирургической резекции опухоли. Медиана выживаемости больных в этой популяции не превышает 24,5 мес.</p><p>В последнее время в лечении ограниченных рецидивных очагов глиобластомы приобретает все большее значение стереотаксическая радиохирургия. Это обусловлено значительным ростом популяции пациентов с глиобластомой, увеличением их продолжительности жизни, а также высокой эффективностью и органосохраняющей спецификой радиохирургического воздействия.</p><p>В нашей клинике стереотаксическое радиохирургическое лечение проводится на аппарате «гамма-нож». Используется высокая доза сфокусированного излучения (за одну или несколько процедур) и резкое сокращение дозы, чем достигается выраженный радиобиологический эффект наряду со щадящим отношением к окружающим опухоль анатомическим структурам.</p><p>В статье приведен клинический пример лечения рецидивирующей глиобластомы на аппарате «гамма-нож», характеризующийся длительным сроком выживания пациента и высоким качеством его жизни.</p></abstract><trans-abstract xml:lang="en"><p>Malignant gliomas consist 15.6 % of all primary brain tumors. Amidst the group of neoplasms, glioblastoma remains the most common primary malignant brain tumor in adults. Mortality of patients with glioblastoma remains the highest among all cancer patients, approaching the absolute. At the present stage, a combined multimodal approach based on adjuvant chemotherapy and radiotherapy after the most safe microsurgical resection of the tumor is considered to be the standard protocol for the treatment of glioblastoma, but the median survival of patients does not exceed 24.5 months.</p><p>Recently, the role of stereotactic radiosurgery in the treatment of glioblastoma recurrence is increasing, which is due to the high efficiency and organ-preserving specifics of the radiosurgical effect. Our clinic uses stereotactic radiosurgery on the gamma knife, which is a successful combination of surgical and radio-oncological treatment technologies, using a high dose of focused radiation (in one or several procedures) and a sharp drop in the dose, which results in a pronounced radiobiological effect, along with a gentle attitude towards adjacent anatomical structures.</p><p>As part of this work, a clinical example of long-term survival with a high quality of life is given in the treatment of recurrent glioblastoma on the gamma knife.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>радиохирургия</kwd><kwd>гамма-нож</kwd><kwd>глиобластома</kwd><kwd>рецидив</kwd><kwd>продолженный рост</kwd></kwd-group><kwd-group xml:lang="en"><kwd>radiosurgery</kwd><kwd>gamma knife</kwd><kwd>glioblastoma</kwd><kwd>relapse</kwd><kwd>continued growth</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Абсалямова О.В., Аникеева О.Ю., Голанов А.В. и др.
