Проведено проспективное исследование с участием 100 пациентов с дегенеративным СПК на поясничном уровне. Все пациенты перенесли интерламинарную декомпрессию содержимого позвоночного канала на симптомной стороне. Критерием эффективности хирургического лечения было уменьшение интенсивности боли, оцененной по визуально-аналоговой шкале и по опроснику Освестри через 1 и 2 года после операции. У пациентов с неудовлетворительными результатами определяли причину неэффективности операции и выявляли факторы риска рецидива СПК.</p></sec><sec><title>Результаты</title><p>Результаты. Через 1 год после операции результат был удовлетворительным у 71 из 100 пациентов, через 2 года после операции – у 67. Причинами неудовлетворительных результатов и рецидивов болевого синдрома у 33 пациентов оказались: рецидив СПК – у 9, появление грыжи диска на уровне операции – у 2, развитие фасеточного синдрома – у 4, развитие клинической нестабильности – у 4, появление боли в противоположной ноге (боли не было до операции) – у 3, развитие отсроченной радикулопатии – у 2, декомпенсация сопутствующих заболеваний – у 9 (коксартроза – у 4, гонартроза – у 3, хронической ишемии нижних конечностей – у 2). При определении факторов риска рецидивов СПК удалось установить их статистически значимую взаимосвязь только с наличием спондилолистеза.</p></sec><sec><title>Заключение</title><p>Заключение. Интерламинарная декомпрессия – эффективный хирургический метод лечения пациентов с дегенеративным СПК на поясничном уровне. Наличие спондилолистеза является фактором риска неэффективности данной операции.</p></sec><sec><title>Конфликт интересов</title><p>Конфликт интересов. Все пациенты подписали информированное согласие на участие в исследовании.</p></sec></abstract><trans-abstract xml:lang="en"><p>The study objective is to assess the effectiveness of interlaminar decompression in patients with degenerative lumbar stenosis.</p><sec><title>Materials and methods</title><p>Materials and methods. A prospective study was conducted among 100 patients with degenerative lumbar stenosis. All patients were operated, interlaminar decompression of the symptomatic side was made. Outcomes were assessed by using a visual analogue scale and Oswestry questionnaire 1 and 2 years after surgery. Among patients with an unsatisfactory result, the cause of the unsatisfactory outcome was studied.</p></sec><sec><title>Results</title><p>Results. One year after surgery a satisfactory result was noted in 71 patients, and 2 years after surgery a satisfactory result was noted in 67 patients. The following causes of unsatisfactory outcome were found in 33 patients: 9 – spinal canal restenosis, 2 – herniated disc at the operation level, 4 – the facet syndrome, 4 – development of clinical instability, 3 – pain in the opposite leg (there was no pain before the operation), 2 – development of delayed radiculopathy, 9 – decompensation of concomitant diseases (4 – coxarthrosis, 3 – gonarthrosis, 2 – chronic ischemia of the lower extremities). Among the risk factors for restenosis, statistically significant relationship was found only with the presence of spondylolisthesis.</p></sec><sec><title>Conclusion</title><p>Conclusion. Interlaminar decompression is an effective surgical option in patients with degenerative lumbar stenosis. The presence of spondylolisthesis is a risk factor for the inefficiency of this operation.</p></sec><sec><title>Conflict of interest</title><p>Conflict of interest.
