Цель исследования – описать клинические случаи гематомы желтой связки у больных с дегенеративным стенозом позвоночного канала на поясничном уровне.

Материалы и методы

Материалы и методы. В клиническом медицинском центре Московского государственного медико-стоматологического университета им. А.И. Евдокимова находились на лечении 3 пациента со стенозом позвоночного канала на поясничном уровне, обусловленным гематомой желтой связки. У всех пациентов наблюдались корешковые боли и нейрогенная хромота вследствие стеноза.

Результаты

Результаты. У всех пациентов выполнили декомпрессию спинного мозга в зоне стеноза с применением заднего доступа. В ходе операции было установлено, что главной причиной сдавления спинного мозга была гематома желтой связки. После декомпрессии у всех пациентов наблюдался регресс корешковых болей, стала возможна ходьба на неограниченные дистанции.

Заключение

Заключение. Гематома желтой связки – крайне редкая находка у пациентов с дегенеративным стенозом позвоночного канала на поясничном уровне, которая может стать главным фактором развития стеноза. Хирургическое лечение позволяет эффективно устранить стеноз и его последствия.

The study objective is to describe the clinical cases of ligamentum flavum hematoma in patients with degenerative lumbar stenosis.

Materials and methods

Materials and methods. Three patients with lumbar stenosis because of ligamentum flavum hematoma were treated in Clinical Medical Center of the A.I. Evdokimov Moscow State University of Medicine and Dentistry. All patients had radicular pain and neurogenic claudication.

Results

Results. Surgical decompression was performed in all cases, intraoperative we discovered ligamentum flavum hematoma, which was the main cause of stenosis. After decompression, all patients were free of pain and claudication.
