На 2-м этапе выполнили разметку доступа, редрессации и резекции ВСА. На 3-м этапе с использованием эндоскопии с высокой точностью при минимальном травмировании выделили ВСА на протяжении, обнаружили ее патологическую петлю, выполнили редрессацию и резекцию ВСА с последующим реанастомозированием.</p></sec><sec><title>Результаты</title><p>Результаты. Объемная скорость кровотока по ВСА составила 320 мл/мин (до операции – 140 мл/мин). В послеоперационном периоде наблюдалась положительная динамика неврологического статуса пациента. При компьютерной ангиографии и триплексном ультразвуковом сканировании подтверждено устранение патологической извитости. Другие варианты операций (через открытые доступы), в отличие от предлагаемого, характеризуются высоким риском травматизации ветвей черепных нервов, подчелюстной и околоушной желез, необходимостью рассечения двубрюшной, шилоподъязычной мышц, сублюксации нижней челюсти и др.</p></sec><sec><title>Заключение</title><p>Заключение. Методика выделения труднодоступных извитостей сонных артерий, разработанная на основании известных ранее эндоскопических манипуляций на сосудах конечностей, с использованием эндоскопии позволяет уменьшить травматичность вмешательства и получить максимальный технический и клинический эффект.</p></sec></abstract><trans-abstract xml:lang="en"><p>The study objective is to describe a clinical case of surgical treatment of a patient with pathological distal tortuosity of the left internal carotid artery (ICA) and present a comprehensive description of a proposed surgical technique, as well as compare advantages and disadvantages of the chosen and alternative surgery types.</p><sec><title>Materials and methods</title><p>Materials and methods. At the 1st stage, we performed computed angiography and triplex scanning of the main head arteries and modelled operative approach. At the 2nd stage, we mapped the ICA approach, redressing and resection. At the 3rd stage, using endoscopic assistance we separated the ICA along its entire length, found its pathological kink, performed ICA redressing and resection with subsequent reanastomosis with high accuracy and minimal injury.</p></sec><sec><title>Results</title><p>Results. ICA blood flow was 320 ml/min (prior to surgery: 140 ml/min). In the postoperative period, positive dynamics of the patient’s neurological status were observed. Computed angiography and triplex scanning confirmed removal of pathological tortuosity. Compared to the proposed technique, other surgery types (open access) are characterized by high risk of injury of the cranial nerve branches, submandibular and parotid glands, necessity of dissection of the digastric, stylohyoid muscles, subluxation of the mandibula, etc.</p></sec><sec><title>Conclusion</title><p>Conclusion. The method of allocation of hard to reach tortuosities of the carotid artery developed based on previously known endoscopic manipulations on the vessels of the extremities using endoscopic assistance allows to decrease injury during the intervention and achieve maximal technical and clinical effect.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>внутренняя сонная артерия</kwd><kwd>патологическая извитость</kwd><kwd>эндоскопическое лечение</kwd><kwd>малоинвазивная хирургия</kwd><kwd>редрессация</kwd></kwd-group><kwd-group xml:lang="en"><kwd>internal carotid artery</kwd><kwd>pathological tortuosity</kwd><kwd>endoscopic treatment</kwd><kwd>minimally invasive surgery</kwd><kwd>redressing</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Ballotta E., Da Giau G., Baracchini C., Manara R.
