В 1-ю группу вошли пациенты, которые прошли хирургическое лечение (n = 31), во 2-ю группу – пациенты, которые прошли консервативное лечение (n = 16). В 1-й группе 15 пациентов перенесли ликворошунтирующую операцию (ЛО), 4 – декомпрессивную трепанацию задней черепной ямки (ДКТ ЗЧЯ), 14 – ЛО в сочетании с ДКТ ЗЧЯ. Критериями эффективности хирургического лечения считали возвращение ясного сознания и/или восстановление конфигурации IV желудочка и четверохолмной цистерны. Результаты оценивали по шкале исходов Глазго.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Возвращение ясного сознания наблюдали на 13-е сутки у 7 из 15 пациентов после изолированной ЛО, т. е. эффективность ЛО составила 47 %. После изолированной ДКТ ЗЧЯ у 2 из 4 пациентов возникла необходимость в установке наружного вентрикулярного дренажа, т. е. эффективность хирургического вмешательства составила 50 %. После ДКТ ЗЧЯ в сочетании с ликворошунтирующими операциями ясное сознание возвратилось у 11 из 14 пациентов в среднем на 4-е сутки. Эффективность данного хирургического подхода составила 79 %. Сочетанное выполнение ДКТ ЗЧЯ и ЛО было на 32 % эффективнее изолированной ЛО и на 29 % эффективнее изолированной ДКТ ЗЧЯ.</p></sec><sec><title>Заключение</title><p>Заключение. У больных со злокачественным инфарктом мозжечка с нарушением сознания и признаками сдавления ствола мозга целесообразно выполнять ЛО в сочетании с ДКТ ЗЧЯ, так как изолированная ЛО не всегда дает необходимый эффект при декомпенсации дислокационного синдрома.</p></sec></abstract><trans-abstract xml:lang="en"><p>The study objective is to evaluate the effectiveness of various methods of surgical treatment of malignant cerebellar infarction.</p><sec><title>Materials and methods</title><p>Materials and methods. There were 47 patients with malignant cerebellar infarction retrospectively analyzed in this study. The group 1 included all patients who underwent surgical treatment (n = 31), the group 2 included all patients who underwent conservative treatment (n = 16). In the group 1, 15 patients underwent isolated ventriculostomy, 4 – posterior fossa decompression (PFD), 14 – combination of ventriculostomy and PFD. The criteria of efficacy of surgery were: restoration of consciousness, restoration of forth ventricle and the quadrigeminal cistern configurations. Results of treatment were assessed according to the Glasgow outcome scale.</p></sec><sec><title>Results</title><p>Results. The recovery of consciousness level was observed in 7 out of 15 patients after isolated ventriculostomy on average 13 day; the efficacy was 47 %. Four patients underwent only PFD. Among them 2 patients needed additional external ventricular drainage installation as 2nd operation. The efficacy was 50 %. After PFD with simultaneous ventriculostomy the recovery of consciousness was observed on 4 day in 11 out of patients. The efficacy was 79 %.
