Н.В. Склифосовского в состоянии средней степени тяжести, с повышенным артериальным давлением до 200/110 мм рт. ст. При компьютерной томографии (КТ) головного мозга обнаружена правосторонняя путаменальная ВМГ объемом 25 см3 с зоной минимального перифокального отека. При КТ-ангиографии выявлена мешотчатая аневризма дистальных отделов правой ЛСА с размером купола 5 мм и шириной шейки 2 мм. Выполнена костно-пластическая трепанация черепа в правой лобно-височно-теменной области. Осуществлен транссильвиевый трансинсулярный доступ к ВМГ, которая была представлена плотными кровяными свертками. При удалении свертков произошел разрыв аневризмы. Выполнен треппинг аневризмы, гематома удалена.</p></sec><sec><title>Результаты</title><p>Результаты. Данное клиническое наблюдение продемонстрировало возможность успешного микрохирургического лечения пациентки с разрывом дистальной аневризмы латеральной ЛСА. При обследовании через 6 мес у пациентки полностью регрессировал парез левой ноги, частично восстановился объем движений левой руки.</p></sec><sec><title>Заключение</title><p>Заключение. Приведенное наблюдение представляет интерес в связи с исключительной редкостью глубинных ВМГ подобной этиологии. Оптимальный метод лечения аневризм ЛСА остается предметом дискуссии ввиду малого числа пациентов с данной патологией и малоизученностью естественного течения заболевания. Пациентам с фузиформными аневризмами показано консервативное лечение и динамическое наблюдение с периодическими церебральными ангиографиями, пациентам с типичными мешотчатыми аневризмами или с наличием ВМГ, имеющих масс-эффект, рекомендовано хирургическое лечение.</p></sec></abstract><trans-abstract xml:lang="en"><p>The study objective is to describe a rare case of putaminal intracerebral hemorrhage (PICH) results from a rupture of the distal lateral lenticulostriate artery (LSA) aneurysm and to summarize currently available information on its diagnosis and treatment.</p><sec><title>Materials and methods</title><p>Materials and methods. A 40-year-old female patient was admitted to the N.V. Sklifosovsky Research Institute of Emergency Medicine. Her arterial pressure was 200/110 mm Hg. Brain computed tomography (CT) revealed a 25 сm3 right-sided PICH with a minimal perifocal edema. CT angiography revealed a saccular aneurysm located in the right distal LSA with the dome of 5 mm and the neck of 2 mm. We performed right frontotemporoparietal osteoplastic craniotomy and used the transsylvian transinsular approach to PICH represented by ruptured while we were removing the blood clots. We performed aneurysm trapping and removed the hematoma.</p></sec><sec><title>Results</title><p>Results. This clinical case demonstrates the possibility of successful microsurgical repair of the distal lateral LSA aneurysm rupture. Six months postoperatively, the patient had complete regression of left leg paresis and partial restoration of left hand functions.</p></sec><sec><title>Conclusion</title><p>Conclusion. This case report appears quite interesting since deep intracerebral hematomas of this etiology are extremely rare. An optimal treatment for LSA aneurysms remains a subject of debate due to the small number of patients with this disorder and limited data on it. Patients with fusiform aneurysms require conservative treatment and dynamic follow-up with periodic cerebral angiography.
