Материалы и методы. В Клинике нервных болезней УКБ № 3 Первого МГМУ им. И.М. Сеченова в 2015 г. находился на лечении пациент с венозно-кавернозной мальформацией мозжечка, которая явилась причиной ишемического инсульта правого полушария мозжечка с геморрагическим пропитыванием. Проведен обзор доступной литературы по данной патологии. Результаты. При поступлении у пациента была клиническая картина поражения мозжечка справа. По данным нейровизуализации определяется ишемический инсульт правого полушария мозжечка с геморрагическим пропитыванием, а также тромбированная венозная ангиома правого полушария мозжечка с небольшим локальным кровоизлиянием вокруг ангиомы. Пациенту выполнено оперативное вмешательство: резекционная трепанация черепа, иссечение сосудистой мальформации правого полушария мозжечка. В гистологическом препарате определяется смешанная кавернозно-венозная мальформация. В послеоперационном периоде отмечался частичный регресс неврологических расстройств, пациент начал передвигаться самостоятельно. Заключение. Факт кровоизлияния из ассоциированной мальформации является очевидным показанием к оперативному лечению. Техника выполнения операции остается предметом дискуссии. Вопрос о риске иссечения сборной вены и риске рецидива КА при ее сохранении не решен. В представленном клиническом примере в ходе операции было проведено радикальное удаление КА и иссечение ВА. Считаем, что при расположении мальформации вне функционально значимых зон мозга возможна коагуляция мозговых вен «головы Медузы», а также коагуляция иликлипирование сборной вены для предупреждения рецидива кавернозной мальформации.</p></abstract><trans-abstract xml:lang="en"><p>Authors present the clinical case of patient with cavernous venous malformation which leaded to ischemic stroke of right cerebellar hemisphere with hemorrhagic transformation. The pathogenesis, features of clinical signs, methods of diagnostics and surgical treatment are discussed.
