Материалы и методы: представлено описание клинического случая острого развития симптомов поперечного поражения спинного мозга у пациента 25 лет, наблюдавшегося в течение 2 лет, со стабильным течением аномалии Киари I типа и сирингомиелии. Было произведено срочное хирургическое вмешательство в объеме краниовертебральной декомпрессии с пластикой твердой мозговой оболочки фрагментом широкой фасции бедра. Результаты: после проведения краниовертебральной декомпрессии в неврологическом статусе наблюдалось увеличение силы мышц конечностей (с полным восстановлением в руках), регресс координаторных нарушений в руках и тазовых нарушений. Контрольное МР-исследование краниовертебрального перехода и спинного мозга, проведенное через 3 мес, показало формирование полноценной большой цистерны и регресс сирингомиелии. Заключение: острое развитие симптомов при аномалии Киари I типа является редкостью и встречается приблизительно в 1% наблюдений, по данным проведенного обзора литературы. Патогенез острого развития симптомов у этих больных до настоящего времени остается неизвестным. О возможности острого развития грубой неврологической симптоматики необходимо помнить в ситуациях, когда пациентам с аномалией Киари I типа и сирингомиелией в качестве лечебной тактики предлагается динамическое наблюдение.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to present the clinical case of acute manifestation of transverse spinal cord damage syndrome in patient 25 years old suffered from Chiari malformation type I and syringomyelia. Material and methods: we present the clinical case of acute transverse spinal cord damage syndrome in patient 25 years old who was follow up examined during 2 years with stable course of Chiari malformation type I and syringomyelia. The emergent operation including craniovertebral decompression with dura mater plasty using fascia lata was performed. Results: the increase of muscle strength in extremities (with full recovery in upper limbs) as well as regress of coordination disturbances in upper limbs and bowel and bladder functions recovery was observed in neurologi cal status after craniovertebral decompression. The control MRI of craniovertebral region and cervical spine performed in 3 months after operation confirmed the formation of adequate cisterna magna and regress of syringomyelia. Conclusion: the acute development of symptoms because of Chiari malformation type I is very rare and occurs approximately in 1% among patients with such pathology according to literature data. The pathogenesis of acute manifestation of symptoms is still not clearly defined.
