Цель исследования. Обзор данных литературы и анализ тактики лечения беременной пациентки в остром периоде САК. Материалы и методы. Проведен обзор данных литературы и клиническое наблюдение пациентки в остром периоде САК на фоне бихориальной, биамниотической беременности на сроке 22-23 нед гестации (анализ медицинской документации с проспективным наблюдением за пациенткой). Результаты. Представлено наблюдение успешного хирургического лечения аневризмы внутренней сонной артерии у пациентки в остром периоде САК на фоне беременности двойней в сроке 22-23 нед с пролонгированием беременности. Выбор метода хирургического лечения обусловлен, с одной стороны, необходимостью выключения из кровотока аневризмы с целью предотвращения рецидива кровоизлияния и осуществления адекватной терапии ангиоспазма, с другой стороны - желанием сохранить жизнь и здоровье матери и 2 младенцев. Заключение. При кровоизлиянии у беременной в результате разрыва аневризмы сосудов головного мозга пациентке показано проведение неотложного нейрохирургического вмешательства, направленного на выключение аневризмы из кровотока, которое может быть выполнено с сохранением беременности.

The subarachnoid hemorrhage (SAH) during pregnancy is rare but extremely dangerous pathology because of high mortality and morbidity in both mothers and children. Objective: to present the treatment strategy of pregnant patient in acute period of SAH and literature review data. Material and methods. The literature review was performed as well as clinical observation of pregnant patient (22-23 weeks of dichorial biamniotic pregnancy) with estimation of medical reports and follow-up results. Results. The successful surgical treatment of patient with dichorial biamniotic pregnancy (22-23 weeks of gestation) suffered from rupture of ICA aneurysm in acute period of SAH was performed. The choice of surgical strategy was determined by necessity of aneurysm elimination and cerebral angiospasm treatment as well as the desire for life surviving of both mother and 2 children. Conclusion. The urgent surgical treatment of ruptured aneurysm which allows keeping the pregnancy must be perform in pregnant patients with ruptured aneurysm in acute period of SAH.
