Крупные мультицентровые исследования не смогли продемонстрировать преимущества хирургической реваскуляризации головного мозга как в профилактике ишемического инсульта с ипсилатеральной стороны, так и в лечении когнитивного дефицита. Сообщения последних лет демонстрируют повышенный интерес к изучению эффективности и безопасности двуствольного экстра-интракраниального анастомоза в лечении пациентов с хронической окклюзией внутренней сонной артерии, однако такие публикации содержат в основном единичные случаи лечения и не носят систематический характер. Целью данной работы являются анализ и систематизация доступных данных литературы по безопасности, эффективности и технике выполнения реваскуляризации головного мозга путем формирования двуствольного экстра-интракраниального микроанастомоза при хронической окклюзии внутренней сонной артерии. </p></abstract><trans-abstract xml:lang="en"><p>Chronic occlusion of the internal carotid artery may be a cause of hemodynamic ischemic stroke and may also manifest itself by progressive decline in cognitive function. Large multicenter studies have failed to demonstrate the benefits of surgical cerebral revascularization both in the prevention of ischemic stroke on the ipsilateral side and in the treatment of cognitive deficits. Reports in recent years indicate high efficacy and safety of double-barrel EC–IC bypass in the treatment of patients with chronic internal carotid artery occlusion; however, such publications are few and contain mostly single cases of treatment.
