В современных вооруженных конфликтах огнестрельные ранения головы остаются актуальной проблемой. Огнестрельные черепно-мозговые ранения срединной линии (ОЧМР СЛ) составляют около 5 % всех черепно-мозговых ранений и характеризуются высокой летальностью. </p><p>Цель исследования – определить анатомо-топографические особенности ОЧМР СЛ, оценить результаты хирургического лечения и разработать оптимальную тактику лечения. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализирована одноцентровая ретроспективная серия клинических случаев. Изучены результаты хирургического лечения 14 пациентов с ОЧМР СЛ, госпитализированных в Городскую больницу №2 г. Белгорода в 2024–2025 гг. Проводились клинико-лабораторные исследования, компьютерная томография, спиральная компьютерно-томографическая ангиография.</p></sec><sec><title>Результаты</title><p>Результаты. Выделено 6 видов ОЧМР СЛ по локализации входного отверстия. У всех пациентов выявлены переломы костей свода и основания черепа, очаги ушибов головного мозга. Внутримозговые гематомы наблюдались в 77 % случаев. Летальность составила 7 %. </p></sec><sec><title>Заключение</title><p>Заключение. ОЧМР СЛ – сложная группа повреждений, требующая специализированной нейрохирургической помощи. Современные технологии и совершенствование хирургических техник позволяют улучшить результаты лечения. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In modern armed conflicts, gunshot wounds to the head remain a relevant problem. Gunshot midline traumatic brain injury (GMTBI) account for approximately 5 % of all traumatic brain injury (TBI) and characterized by high mortality.</p></sec><sec><title>Aim</title><p>Aim. To determine the anatomical and topographical features of GMTBI, evaluate surgical treatment outcomes, and develop optimal treatment strategy. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. Single-center retrospective clinical case series was analyzed, including surgical treatment outcomes of 14 patients with GMTBI admitted to City Hospital No. 2, Belgorod, in 2024–2025. All patients underwent clinical and laboratory examinations as well as CT and CT angiography were performed.</p></sec><sec><title>Results</title><p>Results. Six types of GMTBI were identified based on the location of the entry wound. All patients had fractures of the skull vault and base as well as contusion foci of the brain. Intracerebral hematomas were observed in 77 % of cases. Mortality was 7 %. </p></sec><sec><title>Conclusion</title><p>Conclusion. GMTBI are a complex group of injuries requiring specialized neurosurgical care.
