Нетравматическая оптическая нейропатия – поражение зрительного нерва, обусловленное постепенно прогрессирующим его сдавлением опухолью или иным гиперпластическим процессом, что в итоге приводит к атрофии и стойкому зрительному дефициту. Это обусловливает необходимость декомпрессии зрительного нерва, однако не существует способов прогнозировать результат.</p><p>Цель исследования – определить факторы, влияющие на динамику зрительных нарушений после декомпрессии, и их прогностическую значимость.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы результаты хирургического лечения 64 пациентов с нетравматической оптической нейропатией. Все пациенты обследованы в объеме визометрии, периметрии и офтальмоскопии. Изменения поля зрения классифицировали по 7 степеням выраженности. Для определения предиктивной значимости количественных переменных в отношении благоприятного результата операции выполнен ROC-анализ. Впоследствии полученные пороговые значения были использованы для определения ключевых предикторов благоприятного исхода и построения прогностической модели методом многофакторной логистической регрессии.</p></sec><sec><title>Результаты</title><p>Результаты. Наиболее значимые предикторы улучшения зрения после операции – степень изменения поля и степень остроты зрения до операции. Данные показатели независимо от других факторов обеспечивали точность прогноза более 80 %. Наименее значимую роль в прогнозе улучшения зрения сыграла длительность анамнеза. Итоговая регрессионная модель включила 3 предиктора: длительность зрительных нарушений менее 12 мес (3 балла), остроту зрения &gt;0,1 (4 балла) и степень изменения поля зрения &lt;5 (5 баллов). При максимуме баллов расчетная вероятность улучшения зрения составляет 93,5 %. Модель является статистически значимой (χ2‑критерий Вальда; p &lt;0,001) и согласуется с фактическими данными (критерий Хосмера–Лемешова; p = 0,504). Разработанная модель объясняет 60,8 % вариабельности исхода, а точность прогноза составляет 90,5 %.</p></sec><sec><title>Заключение</title><p>Заключение. Выявлены факторы, влияющие на динамику зрительных функций у пациентов с компрессионной нетравматической оптической нейропатией после микрохирургической декомпрессии зрительного нерва. Обнаружение данных предикторов позволило разработать балльную шкалу для оценки вероятности улучшения зрения после операции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Non-traumatic optic neuropathy is damage of the optic nerve caused by its progressive compression by a tumor or other hyperplastic process which leads to atrophy and constant visual impairment. Therefore, the optic nerve needs to be decompressed but there are no methods of predicting the results of decompression.</p></sec><sec><title>Aim</title><p>Aim. To identify factors affecting dynamics of visual impairment after decompression and their prognostic value.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The results of surgical treatment of 64 patients with non-traumatic optic neuropathy were analyzed. All patients were examined using visometry, visual filed test, and ophthalmoscopy. Visual field changes were classified per 7 grades of severity. Determination of predictive significance of quantitative variables for favorable surgical results was performed using ROC analysis. The obtained threshold values were used to identify key predictors of favorable outcome and to develop a prognostic model employing multivariable logistic regression.</p></sec><sec><title>Results</title><p>Results. The most significant predictors of improvement in vision after surgery are severity of visual field change and visual acuity prior to surgery. These characteristics gave prognostic accuracy of more than 80 % independently of other factors. The least significant characteristic for predicting visual improvement was duration of anamnesis. The final regression model included 3 predictors: duration of visual impairment less than 12 months (3 points), visual acuity &gt;0.1 (4 points), and degree of visual filed change &lt;5 (5 points). For maximal points, calculated probability of improvement in vision is 93.5 %. The model is statistically significant (Wald χ2 test; p &lt;0.001) and complies with factual data (Hosmer–Lemeshow test; p = 0.504). The developed model explains 60.8 % of outcome variability, and accuracy of prognosis is 90.5 %.</p></sec><sec><title>Conclusion</title><p>Conclusion. Factors affecting dynamics of visual function in patients with compression non-traumatic optic neuropathy after microsurgical decompression of the optic nerve were identified.
