Исследование проведено на базах ГБУЗ «НИИ СП им. Н. В. Склифосовского», ФГБНУ «Российский центр неврологии и нейронаук», ГБУЗ «ГКБ им. С. С. Юдина ДЗМ» с 1 января 2009 г. по 31 декабря 2023 г. В анализ включены 45 пациентов с дистальными аневризмами перикаллезной и средней мозговой артерий. С учетом методики хирургического лечения пациенты разделены на 2 группы: в 1‑ю группу (n = 21) вошли пациенты, оперированные с использованием безрамной нейронавигации, из них 10 (47,6 %) – с дистальными аневризмами средней мозговой артерии, 11 (52,4 %) – с дистальными аневризмами перикаллезной артерии. Во 2‑ю группу (n = 24) вошли пациенты, оперированные без использования безрамной нейронавигации, из них 10 (41,7 %) – с дистальными аневризмами средней мозговой артерии, 14 (58,3 %) – с дистальными аневризмами перикаллезной артерии.</p></sec><sec><title>Результаты</title><p>Результаты. Использование безрамной нейронавигации помогает в поиске дистальных аневризм на основном этапе операции, уменьшает временной интервал от разреза до клипирования дистальной аневризмы (p = 0,0001), уменьшает длительность операции (р = 0,0001), однако требует больше времени для планирования доступа (p = 0,008). Безрамная навигация помогает определить оптимальные размеры и локализацию краниотомии для наиболее прямой и безопасной траектории подхода к дистальной аневризме (p = 0,008), снижает риск интраоперационного повреждения лобной пазухи (p = 0,025). У пациентов, у которых клипирование дистальной аневризмы осуществлялось с использованием безрамной навигации, отмечалось уменьшение частоты развития нового неврологического дефицита (p = 0,0001), послеоперационных (p = 0,025) и системных осложнений (p = 0,005), повторных операций (p = 0,002).</p></sec><sec><title>Заключение</title><p>Заключение. Использование безрамной нейронавигации уменьшает время поиска дистальной аневризмы и длительность операции, позволяет сократить площадь краниотомии и предотвратить интраоперационное повреждение лобной пазухи, статистически значимо снижает риск развития послеоперационного неврологического дефицита и улучшает клинические исходы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare results of surgical treatment of patients with distal aneurysms of the middle cerebral and pericallosal arteries who underwent surgery with frameless neuronavigation and without it.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was performed at the N. V. Sklifosovsky Research Institute for Emergency Medicine, Russian Сenter of Neurology and Neurosciences, Moscow City Hospital named after S. S. Yudin between January 1st 2009 and December 31st 2023. Analysis included 45 patients with distal aneurysms of the pericallosal and middle cerebral arteries. Taking into account surgical technique, the patients were divided into 2 groups: the 1st group (n = 21) included patients who underwent surgery with frameless neuronavigation; among them 10 (47.6 %) patients had distal aneurysms of the middle cerebral artery, 11 (52.4 %) patients had distal aneurysms of the pericallosal artery. The 2nd group (n = 24) included patients who underwent surgery without frameless neuronavigation; among them 10 (41.7 %) patients had distal aneurysms of the middle cerebral artery, 14 (58.3 %) patients had distal aneurysms of the pericallosal artery.</p></sec><sec><title>Results</title><p>Results. The use of frameless neuronavigation helps in the search for distal aneurysms during the main stage of the surgery, decreases time between dissection and clipping of a distal aneurysm (p = 0.0001), decreases operative time (р = 0.0001) but requires more time for access planning (p = 0.008). Frameless neuronavigation helps to determine optimal size and location of craniotomy for the most direct and safe trajectory to the distal aneurysm (p = 0.008), decreases the risk of intraoperative injury of the frontal sinus (p = 0.025). Patients, in whom distal aneurysm clipping was performed using frameless navigation, had lower rate of neurologic deficit (p = 0.0001), postoperative (p = 0.025) and systemic (p = 0.005) complications, repeat surgeries (p = 0.002).</p></sec><sec><title>Conclusion</title><p>Conclusion.
