Инвазивный мониторинг электрической активности головного мозга (инвазивная электроэнцефалография (ЭЭГ)) может иметь ряд преимуществ в диагностике раннего повреждения и отсроченной ишемии мозга на фоне церебрального сосудистого спазма в виде большей чувствительности и меньшего количества артефактов по сравнению с неинвазивным (скальповым) ЭЭГ-мониторингом. Однако в настоящее время отсутствуют стандарты установки инвазивных электродов и регистрации записей инвазивной ЭЭГ. Мы представляем клинический опыт применения инвазивного продленного ЭЭГ-мониторинга у пациентов в остром периоде разрыва церебральных аневризм.</p><p>Цель исследования - разработать методику проведения инвазивного ЭЭГ-мониторинга у больных с разрывом церебральных аневризм и оценить особенности выявленных электрографических изменений.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Представлена серия клинических наблюдений: 6 пациентов, оперированных в остром периоде нетравматического субарахноидального кровоизлияния, которым проводили одновременный скальповый и инвазивный ЭЭГ-мониторинг.</p></sec><sec><title>Результаты</title><p>Результаты. Выявленные по данным ЭЭГ патологические изменения коррелировали с развитием зон ишемии головного мозга. Изменения, зафиксированные при инвазивной ЭЭГ, были более выраженными и опережали по времени возникновения данные скальпового мониторинга.</p></sec><sec><title>Заключение</title><p>Заключение. Информативность инвазивного ЭЭГ-мониторинга в диагностике отсроченной ишемии мозга и бессудорожной эпилептиформной активности выше, чем у скальпового ЭЭГ-мониторинга.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Invasive monitoring of electrical activity of the brain (invasive electroencephalography (EEG)) can have a number of advantages in diagnosis of early damage and delayed ischemia of the brain during cerebral vascular spasm due to its higher sensitivity and lower number of artifacts compared to noninvasive (scalp) monitoring. However, there are no standards of invasive electrode placement and registration of invasive recording. We present clinical experience of invasive prolonged EEG monitoring in patients during the acute period of cerebral aneurysm rupture.</p></sec><sec><title>Aim</title><p>Aim. To develop technique of invasive EEG monitoring in patients with cerebral aneurysm rupture and to evaluate characteristics of the observed electrographic changes.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Series of clinical observations are presented: 6 patients who were operated on in the acute period of nontraumatic subarachnoid hemorrhage and who underwent simultaneous scalp and invasive EEG monitoring.</p></sec><sec><title>Results</title><p>Results. Pathologic changes observed during EEG correlated with development of areas of cerebral ischemia. Changes registered during invasive EEG were more pronounced and appeared earlier than during scalp monitoring.</p></sec><sec><title>Conclusion</title><p>Conclusion.
