Механические осложнения (нестабильность винтовой фиксации и миграция металлоконструкций), связанные с дорзальной инструментальной фиксацией после травмы шейного отдела позвоночника, составляют от 2 до 5 %. Миграция имплантатов, в частности штанг, – явление редкое и потенциально опасное. Миграция металлоконструкций в послеоперационный период может приводить к серьезным неврологическим последствиям, требующим своевременного вмешательства.</p><p>Цель работы – представить и проанализировать клинический случай нестабильности и миграции металлоконструкции после винтовой фиксации сегментов шейного отдела позвоночника (C1–C2–C3–C4) пациента с травматическим повреждением позвонков С2 и С3, а также сделать обзор литературы для определения факторов риска и методов профилактики подобных осложнений.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведение многоуровневой винтовой фиксации шейного отдела позвоночника пациенту с травматическим повреждением позвонков С2 и С3. Оценка состояния пациента и имплантатов при помощи клинического осмотра и компьютерной томографии. Обзор источников литературы, посвященной миграции металлоконструкций после спондилосинтеза.</p></sec><sec><title>Результаты</title><p>Результаты. Через 4 года после выполненной операции (винтовой фиксации сегментов шейного отдела позвоночника C1–C2– C3–C4 после травматического повреждения позвонков С2 и С3) выявлена миграция штанги в заднюю черепную ямку (Fossa cranii posterior), возникшая из-за нестабильности металлоконструкции и приведшая к неврологическим последствиям. Проведено повторное хирургическое вмешательство, в результате которого металлоконструкция и ее части были удалены. Обзор литературы показал, что такие случаи редки, и позволил выявить особенности ведения таких пациентов и возможные причины миграции металлоконструкций, включая недостаточную фиксацию и технические ошибки. Наш опыт клинического наблюдения и обзор позволяют сформировать ряд рекомендаций: прежде всего требуется внимательный подход к выбору методов и инструментов фиксации и к послеоперационному мониторингу, которые должны стать частью профилактики миграций металлоконструкций.</p></sec><sec><title>Заключение</title><p>Заключение. Редкие случаи нестабильности и миграции металлоконструкции после многоуровневой винтовой фиксации шейного отдела позвоночника требуют индивидуального подхода к лечению и диагностике. Проведение спондилосинтеза с точным соблюдением методологии операции, повышение осведомленности и строгий контроль послеоперационного периода помогут снизить риск осложнений и улучшить результаты лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The rate of mechanical complications (screw system instability and hardware migration) associated with dorsal instrumental fixation of the cervical spine after injury ranges between 2 and 5 %. Implant migration, particularly of rods, following dorsal screw fixation of the cervical spine is a rare but potentially dangerous complication. Such complications can lead to severe neurological consequences and requires timely intervention.</p></sec><sec><title>Aim</title><p>Aim. To analyze a clinical case of instability and migration of metal hardware following screw fixation of the C1–C2–C3–C4 segments in a patient with traumatic fractures of the C2 and C3 vertebra, and to review literature to identify risk factors and methods for preventing such complications.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study includes one clinical case of a patient who experienced complications – implant migration – after multilevel fixation of the cervical spine. Clinical examination, computed tomography (CT) were used to assess the condition of the patient and implants, along with a review of the literature on this issue.</p></sec><sec><title>Results</title><p>Results. The patient was diagnosed with instability of the metal hardware and migration of the rod into the posterior cranial fossa (Fossa cranii posterior) causing neurologic symptoms. A second surgical intervention was performed to remove the metal hardware. The literature review revealed that such cases are rare and require careful approach to selection of fixation methods and postoperative monitoring.</p></sec><sec><title>Discussion</title><p>Discussion. The analysis of the clinical case and the literature revealed possible causes of migration, including inadequate fixation and technical errors. Recommendations on prevention of such complications and patient management are proposed.</p></sec><sec><title>Conclusion</title><p>Conclusion. Rare cases of instability and migration of the metal hardware following multilevel screw fixation of the cervical spine require personalized approach to diagnosis and treatment.
