Оптимальной стратегией лечения КГО считают хирургическую резекцию, основная цель которой – тотальное удаление образования с наилучшим функциональным и косметическим результатом. Для удаления КГО применяют различные варианты доступов в зависимости от размера и локализации образования, специализации и предпочтений оперирующего хирурга.</p><p>В статье представлены три клинических наблюдения пациентов с КГО, проходивших лечение в Федеральном центре мозга и нейротехнологий ФМБА России. В двух случаях резекция КГО выполнена с помощью модифицированного орбитозигоматического доступа, в одном случае применен трансназальный эндоскопический доступ.</p><p>Вопрос выбора хирургического доступа к образованиям орбиты остается предметом дискуссий среди офтальмологов, нейрохирургов и челюстно-лицевых хирургов. Хирургическое лечение КГО требует дополнительных клинических и анатомических исследований для систематизации хирургических методов с учетом широкого выбора доступов, специфичности анатомии орбиты, локализации КГО, специализации хирурга и индивидуальных особенностей пациента.</p></abstract><trans-abstract xml:lang="en"><p>Cavernous hemangioma of the orbit (CHO) is the most common benign vascular malformation in adults. Surgical resection is considered the optimal treatment strategy for CHO with the aim to totally remove the malformation with the best functional and cosmetic results. During CHO resection, different types of approaches are used depending on the size and location of the malformation, specialization and preferences of the surgeon.</p><p>The article presents 3 clinical cases of patients with CHO who underwent treatment at the Federal Center of Brain Research and Neurotechnologies of the Federal Medical and Biological Agency (FCBRN of FMBA of Russia). In two cases, the CHO was resected using a modified orbitozygomatic approach, in one case a transnasal endoscopic approach was applied. The choice of surgical approach to orbital neoplasms continues to be a topic of discussion between ophthalmologists, neurosurgeons and maxillofacial surgeons.
