Определить угловые параметры операционной раны и направления осмотра труднодоступных сегментов артериального круга большого мозга из орбитозигоматического доступа. Материалы и методы. Исследование было выполнено на 30 анатомических препаратах, на которых моделировали орбитозигоматический доступ. Оценивали угловые параметры операционной раны и доступность базального осмотра А1-сегмента передней мозговой и передней соединительной артерий, клиноидного сегмента внутренней сонной артерии, бифуркации средней мозговой артерии, верхушки базилярной артерии, устья передней нижней мозжечковой артерии. Оценка параметров операционной раны складывалась из измерения проекционного, вертикального и горизонтального углов и глубины раны. Доступы проводили на анатомических препаратах c использованием операционного микроскопа МБС-10, стандартного набора нейрохирургических инструментов и высокоскоростного нейрохирургического костного бора. Результаты. Орбитозигоматический доступ уменьшает проекционные углы к труднодоступным сегментам артериального круга большого мозга. Вертикальные операционные углы возрастают для А1-сегмента передней мозговой и передней соединительной артерий в 2 раза, для клиноидного сегмента внутренней сонной артерии в 2-4 раза, для бифуркации средней мозговой артерии в 1,5 раза, для верхушки базилярной артерии в 3-8 раз по сравнению с птериональным доступом. При орбитозигоматическом транскавернозном доступе вертикальные углы возрастают для устья передней нижней мозжечковой артерии в 3-4 раза по сравнению с ретросигмовидным доступом и в 1,5 раза по сравнению с орбитозигоматическим доступом. Заключение. Орбитозигоматический доступ является универсальным для подхода к труднодоступным сегментам артериального круга большого мозга, несущих аневризму, её дисекции и клипирования, а также для проведения вариантов транскавернозных доступов и расширенных доступов со стороны дна средней черепной ямки (передней петрозэктомии), темпорополярных экстрадуральных, преаури-кулярных инфратемпоральных и комбинированных постаурикулярных транспетрозальных доступов с транспозицией (или без) внутренней сонной артерии, VII нерва, сигмовидного синуса.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To determine the angular parameters of operative wound and observation directions of difficult accessible segments of circle of Willis via orbitozygomatic approach. Material and methods. The orbitozygomatic approach was simulated on 30 anatomical specimens during our study. The angular parameters of operative wound and accessibility of basal observation of А1 segment of anterior cerebral and anterior communicating arteries, clinoid segment of internal carotid artery and bifurcation of middle cerebral artery as well as basilar tip and orifice of anterior inferior cerebellar artery were estimated. The assessment of operative wound parameters consisted of measurement of projection, vertical and horizontal angles and depth of wound. All approaches were performed on anatomical specimens using operative microscope MBS-10, routine set of neurosurgical instruments and high speed neurosurgical bone drill. Results. The orbitozygomatic approach decreases the projection angles for difficult accessible segments of circle of Willis. The vertical operative angles increased by 2 times for А1 segment of anterior cerebral and anterior communicating arteries, by 2-4 times for clinoid segment of internal carotid artery, by 1,5 times for bifurcation of middle cerebral artery and by 3-8 times for basilar tip comparing with pterional approach. The vertical angles increases by 3-4 times for orifice of anterior inferior cerebellar artery while using orbitozygomatic transcavernous approach comparing with retrosigmoid approach and by 1,5 times comparing with orbitozygomatic approach. Conclusions. The orbitozygomatic approach is the general for achieving of aneurysms located on difficult accessible segments of circle of Willis as well as for aneurysm dissection and clipping as well as for various variants of transcavernous approaches and extended approaches via floor of middle cerebral fossa (anterior petrosectomy) as well as for temporopolar extradural, preauricular infratemporal and combined postauricular transpetrosal apprpaches with transposition (or without) of internal carotid artery, VII nerve and sigmoid sinus.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>орбитозигоматический доступ</kwd><kwd>угловые параметры операционной раны</kwd><kwd>артериальная аневризма</kwd><kwd>orbitozygomatic approach</kwd><kwd>angular parameters of operative wound</kwd><kwd>cerebral aneurysm</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Крылов В.В.
