В статье описан клинический случай формирования множественных метахронных интраорбитальных артериовенозных шунтов у пациента с унивентрикулярным пороком сердца и вторичным эритроцитозом. Лечение первого шунта выполнялось трансартериальным путем и имело положительный клинический эффект в течение 2 мес. Рецидив симптомов был обусловлен формированием нового шунта на той же дренирующей вене, однако в удаленном от прежнего локуса месте. Повторная эмболизация дала положительный эффект на аналогичный срок. Следующий рецидив имел более злокачественное течение – вплоть до вывиха глазного яблока. Выполненная декомпрессия медиальной стенки правой орбиты дала стабильный положительный клинический эффект, который наблюдается до настоящего момента. На основании крайне ограниченных данных литературы о подобных случаях и полученного собственного опыта мы сделали вывод о необходимости индивидуального и мультидисциплинарного подхода к лечению таких пациентов с тщательным послеоперационным наблюдением.</p></abstract><trans-abstract xml:lang="en"><p>   Intraorbital arteriovenous fistulas are rare arteriovenous shunts inside the orbit without connection with the cavernous sinus. This article describes a clinical case of multiple metachronous intraorbital arteriovenous shunts in a patient with univentricular heart disease and secondary erythrocytosis. Transarterial embolization of the first shunt had a positive clinical effect for 2 months. The recurrence of symptoms was caused by the formation of a new shunt on the same draining vein, but in a place remote from the previous locus. Repeated embolization gave a positive effect for the same period. The next relapse had a more malignant course, up to dislocation of the eyeball. The right orbit decompression has given a stable positive clinical effect up to the present moment.
