Введение. Кавернома кавернозного синуса – достаточно редкое доброкачественное новообразование, на долю которого приходится <3 % всех новообразований этой области. Из‑за редкости данной патологии стандартизированный протокол диагностики или лечения не разработан. Для лечения используются хирургические и радиохирургические методы.

Цель исследования – анализ результатов лечения пациентов с каверномами кавернозного синуса, оперированных в ФГАУ «НМИЦ нейрохирургии им. акад. Н. Н. Бурденко» Минздрава России с 2000 по 2022 г. с использованием эндоскопического трансназального доступа.

Материалы и методы

Материалы и методы. Мы представляем собственный опыт лечения 9 пациентов, которым было выполнено трансназальное эндоскопическое удаление каверномы кавернозного синуса.

Результаты

Результаты. В литературе представлено описание в общей сложности около 300 случаев лечения пациентов с каверномами кавернозного синуса, и только в 12 случаях описано эндоскопическое удаление.

В нашей серии в 7 случаях удалось добиться субтотального удаления, в 2 случаях удаление было частичным. Послеоперационный период у всех пациентов протекал без особенностей и без осложнений.

Заключение

Заключение. Эндоскопический трансназальный доступ позволяет выполнить частичное или субтотальное удаление образования без риска травмы черепных нервов и с минимальной травмой мягких тканей, что положительно сказывается на течении послеоперационного периода. В сочетании с радиохирургическими методами возможно достижение удовлетворительных результатов лечения.

Background

Background. Cavernoma of cavernous sinus is a fairly rare benign neoplasm, accounting for less than 3 % of all neoplasms in this area. Due to the rarity of this pathology, a standardized protocol for diagnosis or treatment has not been developed. Surgical and radiosurgical methods are used for treatment.

Aim

Aim. Analysis of the results of treatment of patients with cavernous sinus cavernomas operated at the N. N. Burdenko National Medical Research Center of Neurosurgery from 2000 to 2022 using endoscopic transnasal access.

Materials and methods

Materials and methods. We present our own experience in the treatment of 9 patients who underwent transnasal endoscopic removal of cavernous sinus cavernoma.

Results

Results. A total of about 300 cases of treatment of patients with cavernous sinus cavernomas have been described in the literature, and endoscopic removal has been described in only 12 cases.

As a result, subtotal removal was achieved in 7 cases. In 2 cases, the removal was partial. The postoperative period proceeded in all patients without peculiarities and without complications.

Conclusion

Conclusion. Endoscopic transnasal access allows partial or subtotal removal of the formation without risk of injury to cranial nerves and with minimal soft tissue injury, which has a positive effect on the postoperative period of patients. In combination with radiosurgical methods, it is possible to achieve satisfactory treatment results.
