От 0,5 до 8 % профессиональных музыкантов страдают фокальной дистонией. При неэффективности консервативной терапии рассматривается оперативное лечение. Стереотаксическая хирургия в объеме таламотомии при данном заболевании является опцией и представлена ограниченными сериями наблюдений в мировой литературе.</p><p>Цель работы – продемонстрировать клинический случай таламотомии фокусированным ультразвуком у профессионального гитариста с фокальной дистонией кисти.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Пациент в возрасте 40 лет с 2009 г. страдал от симптомов заболевания в виде спазма пальцев кисти при игре на гитаре. По мере прогрессирования тяжести заболевания, в связи с отсутствием эффекта от консервативной терапии пациент утратил профессиональные навыки и сменил деятельность. После его обращения в клинику для рассмотрения возможности хирургического лечения была предложена стереотаксическая деструкция вентроорального ядра таламуса фокусированным ультразвуком.</p></sec><sec><title>Результаты</title><p>Результаты. Таламотомия позволила полностью устранить дистонию в кисти без осложнений. На 2‑е сутки после операции пациент был выписан в удовлетворительном состоянии. Спустя 1 нед была замечена легкая дизартрия, которая не влияла на ежедневную активность и полностью регрессировала в течение месяца. Через 1 мес пациент смог вернуться к сценической деятельности. Катамнез составил 6 мес с отсутствием каких‑либо симптомов заболевания. </p></sec><sec><title>Заключение</title><p>Заключение. Представлен случай успешной вентрооральной таламотомии с помощью фокусированного ультразвука у пациента с дистонией музыканта.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Approximately from 0.5 to 8 % professional musicians may suffer from focal dystonia. Stereotactic surgery is option if conservative therapy is failed. Thalamotomy has been reported in limited series in the world literature.</p></sec><sec><title>Aim</title><p>Aim. To demonstrate a clinical case of thalamotomy with focused ultrasound in a professional guitarist with focal dystonia of the hand.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A 40‑year‑old patient suffered from spasms of the fingers while playing the guitar since 2009. Patient lost professional skills and changed his activities due to progression and severity of disease. He visited our clinic for surgical treatment, stereotactic ventro‑oral thalamotomy by transcranial focused ultrasound was considered.</p></sec><sec><title>Results</title><p>Results. Thalamotomy provided complete reduction of dystonia without complications. On the 2nd day after the procedure, the patient discharged. In a week mild dysarthria was noticed, which did not affect daily activities and completely regressed within a month. A month later, the patient was able to return to concert performance. The follow‑up period was 6 months with no symptoms of the disease.</p></sec><sec><title>Conclusion</title><p>Conclusion.
