Ввиду редкой встречаемости дистальных аневризм головного мозга публикаций, посвященных эндоваскулярному лечению, крайне мало. В связи с развитием эндоваскулярной хирургии количество успешно вылеченных пациентов с дистальными аневризмами в последнее время увеличивается.</p><p>Цель исследования – определить технические возможности и оценить результаты эндоваскулярного лечения пациентов с дистальными аневризмами головного мозга.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Работа основана на анализе результатов эндоваскулярного лечения 45 пациентов с дистальными аневризмами головного мозга в двух нейрохирургических отделениях федеральных медицинских центров. Из 45 проанализированных случаев 30 были без разрыва аневризмы, 15 – с разрывом.</p></sec><sec><title>Результаты</title><p>Результаты. Эндоваскулярное лечение дистальных аневризм позволило добиться хороших результатов лечения (4–5 баллов по шкале исходов Глазго (ШИГ)) в 97,8 % случаев: 5 баллов по ШИГ – 68,9 %, 4 балла по ШИГ – 28,4 % случаев. Получен 1 (2,2 %) летальный исход. Наиболее частыми методиками эмболизации были эмболизация аневризмы микроспиралями со стент-ассистенцией (37,8 %) и установка потокперенаправляющего стента (28,9 %). Реже применялись эмболизация только микроспиралями (20 %) и окклюзия несущей аневризму артерии (13,3 %). У 3 из 6 пациентов окклюзия несущей аневризму артерии была не запланирована.</p></sec><sec><title>Заключение</title><p>Заключение. Развитие эндоваскулярной хирургии и технических возможностей выполнения операций в дистальном сосудистом русле головного мозга позволяет сформировать мультидисциплинарный подход к выбору оптимального метода выключения дистальной аневризмы из кровотока с учетом современных возможностей открытой хирургии. Это особенно важно для пациентов с разрывом аневризмы в тяжелом состоянии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Due to rare occurrence of distal cerebral aneurysms, there are very few publications devoted to endovascular treatment. Due to current progress in the endovascular surgery, the number of successfully cured patients with distal aneurysms has been increasing recently.</p></sec><sec><title>Aim</title><p>Aim. To determine technical capabilities and evaluate results of endovascular treatment of patients with distal intracranial aneurysms.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The work is based on analysis of data of endovascular treatment among 45 patients with distal cerebral aneurysms in two neurosurgical departments of federal medical centers. Of the 45 analyzed cases 30 were without aneurysm rupture and 15 were with rupture.</p></sec><sec><title>Results</title><p>Results. Endovascular treatment of the distal aneurysms made it possible to achieve good treatment results (4–5 points on the Glasgow Outcome Scale (GOS)) in 97.8 % of cases: 5 points on GOS – in 68.9 %, 4 points on GOS – in 28.4 %. One (2.2 %) fatal outcome was obtained. The most frequent methods of embolization were embolization of aneurysm by coils with stent assistance (37.8 %) and installation of a flow-diverter stent (28.9 %). Embolization with only coils (20 %) or occlusion of parent artery (13.3 %) were used less frequently. In 3 out of 6 patients, the occlusion of parent artery was not planned.</p></sec><sec><title>Conclusion</title><p>Conclusion. Development of endovascular surgery and technical capabilities of performing operations in the distal cerebral arteries has made it possible to form a multidisciplinary approach to choosing the optimal method of shutting off the distal aneurysm from the bloodstream, taking into account the modern possibilities of open surgery.
