Отдаленные ангиографические исследования после эндоваскулярного лечения церебральных аневризм могут демонстрировать нестабильный результат как в виде формирования рецидива, так и в виде повышения степени окклюзии при изначально неполном закрытии мешка аневризмы. Рецидивы церебральных аневризм представляют собой важную медико‑социальную проблему, сопоставимую по рискам кровоизлияния с неоперированной аневризмой.</p><p>Цель исследования – оценка ближайших и отдаленных результатов эндоваскулярного лечения пациентов с церебральными аневризмами, прооперированных с использованием различных внутрисосудистых методик. Материалы и методы. В настоящее исследование включены 167 пациентов (48 мужчин и 119 женщин) с 195 церебральными аневризмами в возрасте от 18 до 75 лет, прооперированных с использованием эндоваскулярных методов лечения с 2013 по 2016 г. на базе нейрохирургического отделения № 3 Российского научно‑исследовательского нейрохирургического института им. проф. А. Л. Поленова – филиала ФГБУ «Национальный медицинский исследовательский центр им. В. А. Алмазова» Минздрава России. Средний возраст больных составил 52 ± 12 лет (от 18 до 75 лет). Выбор метода внутрисосудистого лечения у включенных в исследование пациентов основывался на анатомо‑топографических и морфометрических особенностях аневризмы, а также с учетом периода течения аневризматической болезни.</p></sec><sec><title>Результаты</title><p>Результаты. Согласно данным контрольных ангиографических исследований после внутрисосудистого лечения пациентов с церебральными аневризмами степень окклюзии аневризмы в отдаленном послеоперационном периоде может отличаться от изначально достигнутой. Вероятность рецидива аневризмы после ее изолированной окклюзии спиралями может достигать 13,1 % уже через 6 мес после проведенного оперативного лечения, 31,7 % через 12 мес и 22,3 % через 36 мес, что составляет в среднем 22,5 % (p &lt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Персонализированный подход при выборе оптимальной хирургической тактики для лечения пациентов с церебральными аневризмами позволяет достичь стабильной радикальной окклюзии аневризмы, снизить частоту послеоперационных осложнений и рецидивов аневризм в отдаленном послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Postoperative angiographic examinations after endovascular treatment of cerebral aneurysms may demonstrate unstable results, both in the form of recurrence formation, and in the form of increased degree of occlusion with initially incomplete occlusion of the aneurysm sac. Cerebral aneurysm recurrences represent an important medical and social problem comparable in terms of hemorrhage risks with nonoperated aneurysms.</p></sec><sec><title>Aim</title><p>Aim. To estimate the early and long‑term results of endovascular treatment of patients with cerebral aneurysms ope rated on using different endovascular techniques. Materials and methods. The present study included 167 patients (48 men and 119 women) with 195 cerebral aneurysms aged from 18 to 75 years who were operated on using endovascular techniques at the neurosurgical department No. 3 of the Russian Research Neurosurgical Institute named after Prof. A. L. Polenov from 2013 to 2016. The average age of the patients was 52 ± 12 years, the minimum age was 18 years, and the maximum age was 75 years. The choice of endovascular treatment method in the patients included in the study was based on anatomico‑topographic and morphometric features of aneurysm, as well as taking into account the period of aneurysmal disease course.</p></sec><sec><title>Results</title><p>Results. The study of data of control angiographic examinations after endovascular treatment of patients with cerebral aneurysms showed that in the remote postoperative period the degree of aneurysm occlusion may differ from that initially achieved. The probability of aneurysm recurrence after its isolated occlusion with coils may reach 13.1 % already in 6 months after surgical treatment, 31.7 % after 12 months and 22.3 % after 36 months, averaging 22.5 % (p &lt; 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion.
