Хирургическое лечение сложной сосудистой патологии нервной системы остается актуальной проблемой, требующей применения комбинаций различных технологий. Использование гибридной операционной позволяет сочетать одномоментно или последовательно микрохирургические и эндоваскулярные методы хирургии, что может улучшить исходы лечения.</p><p>Цель работы – улучшение результатов хирургического лечения пациентов с сосудистой патологией головного мозга с помощью возможностей гибридной операционной.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Оперативные вмешательства проводились в гибридной операционной с применением эндоваскулярных и микрохирургических методов лечения следующих сосудистых патологий головного мозга: сложных дуральных фистул, сложных аневризм, артериовенозных мальформаций. В зависимости от характера патологии применяли разные виды хирургического вмешательства: гибридный, комбинированный, этапный.</p></sec><sec><title>Результаты</title><p>Результаты. За 5 лет в гибридной операционной прооперирован 41 пациент, из них 33 – с артериовенозными мальформациями, 6 – со сложными аневризмами, 2 – со сложными дуральными фистулами. Комбинированные вмешательства выполнены у 27 больных, этапные – у 12, гибридные – у 2. В соответствии с модифицированной шкалой Рэнкина (Modified Rankin Scale, mRS) отмечены следующие исходы операций: осложнений не было в случаях сложных аневризм (6 пациентов) – mRS 0 (баллов), а также дуральных фистул (2 пациента) – mRS 0 (баллов); при артериовенозной мальформации головного мозга у 30 больных не было осложнений – mRS 0; возникли разного вида осложнения у 3 – mRS 1.</p></sec><sec><title>Заключение</title><p>Заключение. Совмещение микрохирургических и эндоваскулярных способов лечения в гибридной операционной позволяет сочетать положительные свойства этих методов в соответствии с потребностями хирургического этапа лечения, что улучшает исходы нейрохирургических вмешательств при сложной нейроваскулярной патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Surgical treatment of complex neurovascular pathology remains an important problem requiring use of a combination of various techniques. Utilization of a hybrid operating room allows to simultaneously or sequentially combine microsurgical and endovascular surgical methods which can improve treatment outcomes.</p></sec><sec><title>Aim</title><p>Aim. To improve surgical treatment of patients with cerebrovascular pathology by utilizing the capabilities of a hybrid operating room.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Surgical interventions were performed in a hybrid operating room with a combination of endovascular and microsurgical methods for treating the following cerebrovascular pathologies: complex dural fistulas, complex aneurysms, arteriovenous malformations. The type of surgical intervention – hybrid, combined, staged – was chosen in accordance with the nature of the pathology.</p></sec><sec><title>Results</title><p>Results. In 5 years, 41 patients underwent surgery in the hybrid operating room. Among them, 33 patients had arteriovenous malformations, 6 had complex aneurysms, 2 had complex dural fistulas. Combination interventions were performed in 27 patients, staged – in 12, hybrid – in 2. According to the Modified Rankin Scale (mRS) the following outcomes were observed: no complications in cases of complex aneurysms (6 patients) – mRS 0 (points), as well as in cases of dural fistulas (2 patients) – mRS 0 (points); in arteriovenous malformation, 30 patients did not have any complications – mRS 0; 3 patients had complications (of different types) – mRS 1.</p></sec><sec><title>Conclusions</title><p>Conclusions.
