Объем удаления глиом головного мозга влияет на исход хирургического лечения. Повышение радикальности вмешательства за счет использования различных методик приводит к увеличению сроков выживаемости и рецидива. Работа посвящена первичной оценке эффективности применения методики интраоперационной магнитно-резонансной томографии при удалении глиом головного мозга.</p><p>Цель – обобщить первый опыт использования и оценить эффективность методики высокопольной интраоперационной магнитно-резонансной томографии в хирургии глиальных опухолей головного мозга.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены 9 больных с глиомами различной степени злокачественности, находившихся на лечении в ФГБУ «Федеральный центр мозга и нейротехнологий» ФМБА России. Проведен проспективный анализ выполненных 9 пациентам оперативных вмешательств с применением методики интраоперационной магнитно-резонансной томографии, а также результатов нейровизуализации.</p></sec><sec><title>Результаты</title><p>Результаты. У всех больных на интраоперационной магнитно-резонансной томографии выявлена остаточная опухолевая ткань различного объема. Во всех случаях выполнена дополнительная резекция остаточной опухоли. Несмотря на увеличение продолжительности операции и транспортировку пациента из операционной в процедурную магнитно-резонансной томографии и обратно, осложнений не возникало. По мере накопления опыта в проведении интраоперационной магнитно-резонансной томографии сокращалось время исследования: среднее время сканирования составило 45 мин.</p></sec><sec><title>Заключение</title><p>Заключение. Высокопольная интраоперационная магнитно-резонансная томография с успехом может быть использована в хирургии глиом головного мозга. Методика позволяет увеличить радикальность удаления опухоли без увеличения риска осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Volume of glioma resection positively correlated with treatment results. Advance in extent of resection due to various additive methods leads to prolonged overall survival and delays progression. Our aim was to evaluate the value of intraoperative magnetic resonance imaging.</p><p>Objective – to present the first experience of using intraoperative magnetic resonance imaging and evaluate the effectiveness and safeness of this technique in surgery of glial brain tumors.</p></sec><sec><title>Material and methods</title><p>Material and methods. Prospective analysis of surgical interventions performed using the intraoperative magnetic resonance imaging and the results of neuroimaging in 9 patients with different grade brain gliomas treated in Federal Brain and Neurotechnology Center was carried out.</p></sec><sec><title>Results</title><p>Results. In all patients we detect variable residual tumor volume after first resection. Additional resection was performed in all cases after the intraoperative magnetic resonance imaging. Mean scan time were 45 minutes overall time for scan were decreasing as we gain experience in using intraoperative magnetic resonance imaging.</p></sec><sec><title>Conclusion</title><p>Conclusion. Intraoperative high-field intraoperative magnetic resonance imaging can be successfully used in the surgery of brain gliomas.
