Повреждение твердой мозговой оболочки – достаточно частое явление у пострадавших с позвоночно-спинномозговой травмой. Одна из актуальных проблем – профилактика послеоперационной ликвореи, а также герметизация шва твердой мозговой оболочки.</p><p>Цель работы – определить оптимальную тактику хирургического лечения пострадавших с повреждением твердой мозговой оболочки при переломах грудного и поясничного отделов позвоночника.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Использованы и статистически обработаны данные клинического и инструментального (лучевая диагностика) обследования 167 больных, оперированных по поводу переломов грудного и поясничного отделов позвоночника, сопровождающихся травматическим стенозом позвоночного канала. Основная группа: 55 пострадавших с разрывами твердой мозговой оболочки, верифицированными интраоперационно, контрольная – 112 пациентов без повреждения твердой мозговой оболочки. Всем больным проведена ламинэктомия позвоночника на уровне переломов и транспедикулярная фиксация.</p></sec><sec><title>Результаты</title><p>Результаты. Разрыв твердой мозговой оболочки выявлен у 32,9 % пациентов с переломами грудного и поясничного отделов позвоночника. У 21,8 % из них отмечено ущемление спинного мозга или его корешков в местах разрыва твердой мозговой оболочки. Этот факт нужно учитывать при выполнении декомпрессии и редукционного маневра для предотвращения дополнительных травм невральных структур. Восстановление целостности твердой мозговой оболочки путем прямого ушивания выполняли 33 (60,0 %) пациентам. Средний размер дефекта твердой мозговой оболочки: 13,2 ± 7,4 мм2. У 13 (23,6 %) больных (средний размер дефекта твердой мозговой оболочки 27,5 ± 6,3 мм2) провели пластику дефекта: фрагментом трупной твердой оболочки – у 2 пациентов, имплантатами «Реперен» – у 5, Durepair – у 6 пострадавших. В 9 наблюдениях (16,4 %) восстанавливали герметичность твердой мозговой оболочки без ушивания – методом «сэндвич»-герметизации (средний размер дефекта ТМО – 5,0 ± 2,6 мм2). У 21 больного для герметизации использовали биологические клеевые композиции.</p><p>Послеоперационная раневая ликворея в основной группе произошла у 5 (9,1 %) из 55 пациентов. Чаще (23,1 % больных) раневая ликворея выявлялась после пластики твердой мозговой оболочки, в группе с прямым ушиванием твердой мозговой оболочки это происходило реже (6,1 %). Послеоперационная ликворея не отмечалась при небольших дефектах твердой мозговой оболочки (≤3 мм2) и при ее повреждении в области манжетки корешка. Больным c послеоперационной раневой ликвореей дополнительная герметизация шва твердой мозговой оболочки клеевыми композициями не проводилась. Нагноение послеоперационной раны выявлено в основной группе у 4 (7,3 %), в контрольной группе – у 6 (5,4 %) пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Применение клеевых композиций для герметизации швов твердой мозговой оболочки – эффективный метод профилактики послеоперационной ликвореи. Герметизация шва твердой мозговой оболочки коллагеновой губкой не предотвращает развитие раневой ликовреи.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The dura mater tear are quite common in patients with thoracic and lumbar fractures. Prevention and management of cerebrospinal fluid leakage and sealing of the dura mater suture is an important stage in the treatment of such patients.</p></sec><sec><title>Objective</title><p>Objective: to find an optimal surgical tactics for patients with fractures of the thoracic and lumbar spine and dura mater tear.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. This study included 167 patients operated on for fractures of the thoracic and lumbar spine with concomitant traumatic spinal canal stenosis. We analyzed their clinical data and results of instrumental examination. All patients underwent laminectomy at the level of their fractures and transpedicular fixation. The main group included 55 patients with dura mater tear, whereas the control group comprised 112 patients without dura mater tear.</p></sec><sec><title>Results</title><p>Results. Dura mater tear was found in 32.9 % of patients with fractures of the thoracic and lumbar spine. Of them, 21.8 % had compression of the spinal cord or nerve roots at the sites of dura mater tear. This fact should be taken into account when performing decompression and the reduction maneuver to prevent additional injuries to the neural structures. Thirty-three (60.0 %) patients underwent direct suturing aimed to restore the dura mater integrity. Their mean size of the dura mater tear was 13.2 ± 7.4 mm2. Thirteen patients (23.6 %) with larger dural tear (27.5 ± 6.3 mm2) underwent their repair using either a fragment of dura mater from a deceased donor (n = 2), Reperen implants (n = 5), or Durepair patches (n = 6). In 9 patients (16.4 %), the integrity of dura mater was restored without suturing (the «sandwich»-sealing method) (mean size of the dura mater defect 5.0 ± 2.6 mm2). Twenty-one patients had additional sealing of dura mater suture using bioglue.</p><p>Postoperative wound cerebrospinal fluid leakage was registered in 5 out of 55 patients from the main group. Cerebrospinal fluid leakage was most common in patients who had undergone dura mater repair with implants (23.1 %), while those who had undergone direct dura mater suturing were less likely to develop it (6.1 %). No cerebrospinal fluid leakage was observed in patients with small defects (≤3 mm2) or in those whose dural tears were located at the nerve root cuffs. Patients with postoperative cerebrospinal fluid leakage had no additional sealing of dura mater suture using bioglue.</p><p>Postoperative wound infection was registered in 4 (7.3 %) patients from the main group and 6 (5.4 %) patients from the control group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Sealing of dura mater sutures with glue compositions is an effective method to prevent postoperative cerebrospinal fluid leakage.
