Провести сравнительный анализ эффективности консервативного и хирургического лечения пациентов со злокачественной формой ишемического инсульта Материал и методы. Проведен анализ результатов обследования и лечения 63 больных со злокачественным течением массивного ишемического инсульта (МИИ). У всех больных поперечная дислокация срединных структур головного мозга превышала 7 мм в первые 48 ч от начала заболевания. Консервативное лечение осуществляли 42 больным - 1-я группа. У 21 больного консервативное лечение было дополнено проведением декомпрессивной краниотомии (ДКТ) на стороне пораженного полушария - 2-я группа. У 12 оперированных пациентов проводили мониторинг внутричерепного давления (ВЧД). Результаты. Летальность у больных 1-й группы составила 100%, у больных 2-й группы - 48% (p&lt;0,05). У оперированных больных, которым проводили мониторинг ВЧД, определили 3 типа динамики ВЧД: 1. Нормальное ВЧД, не требующее коррекции - тип 1; 2. Постепенное повышение ВЧД до 20 мм рт.ст. и более, поддающееся коррекции при помощи консервативной терапии - тип 2; 3. Постепенное повышение ВЧД до 20 мм рт.ст. и более с развитием рефрактерной ВЧГ - тип 3. Заключение. Проведение ДКТ больным со злокачественным течением МИИ с поперечной дислокацией срединных структур головного мозга равной 7 мм и более в первые 48 ч после развития инсульта сопровождается снижением летальности более чем в 2 раза. Проведение мониторинга ВЧД у данной категории больных после выполнения ДКТ позволяет диагностировать внутричерепную гипертензию и определить тактику ее коррекции</p></abstract><trans-abstract xml:lang="en"><p>Objective. To conduct the comparative efficacy analysis of conservative and surgical treatment of patients with malignant course of massive ischemic stroke Material and methods. The analysis of examination and treatment results of 63 patients with malignant course of massive ischemic stroke was performed. All patients had midline shift more than 7 mm in first 48 hours from disease beginning. All patients were divided into two groups: 1st group - conservative treatment (42 patients) and 2d group - patients with decompressive craniotomy (12 patients - intracranial pressure (ICP) monitoring). Results. The lethality among patients of 1st group was 100% comparing with 48% among patients of 2d group (p&lt;0,05). The 3 types of ICP hemodynamics at operated patients (with ICP monitoring) were determined: 1. Normal ICP without any requirements for correction - 1 type; 2. Progressive increase of ICP up to 20 mmHg and more, which is manageable by conservative therapy - 2 type; 3. Progressive increase of ICP up to 20 mmHg and more with development of refractory intracranial hypertension - 3 type. Conclusion. The surgical treatment (decompressive trepanation) at patients with malignant course of massive ischemic stroke with midline shift more than 7 mm during first 48 hours since disease beginning is accompanied by more than 2-fold lethality decrease.
